DoT Physicals (DoT med card)

DoT physical exams can usually be scheduled same day by calling the office or going to hss.janeapp.com 

Walk ins are accepted if no other patient is currently being seen but scheduling ahead is preferred.
 
If you can, please bring the printed off form (MCSA - 5875) with the questionnaire and personal information filled out OR come to the appointment 10 minutes ahead of time to fill it out in the office.

Loose fitting clothing is preferred but not necessary for the exam.

Make sure to bring:
- Photo ID (Driver's License)
- Work reimbursement form (if available)

Also Bring: (if necessary)
- Cardiology clearance
- Orthopedic clearance
- Eye glasses and hearing aids 
- Stress testing results
- Sleep apnea testing results
- Previous DoT exam results (if available, not necessary)

Frequently Asked Questions
Is there drug testing?
  - No, there is a urine screen as a component of the exam but it only screens for protein, sugar, and blood in the urine to look for liver, kidney, or blood sugar diseases. 
How long does it take to get the results?
  - You will be given the results same day at the completion of the exam and I submit the results of all the exams to the national registry.
 

FAA Basic Med Exam

FAA basic med exams can usually be scheduled same day by calling the office at (651) 409-2056 or going to hss.janeapp.com.

Walk ins are accepted if no other patient is currently being seen but scheduling ahead is preferred.  

If you can, please bring the printed off form (FAA 8700-2) with the questionnaire and personal information filled out OR come to the appointment 10 minutes ahead of time to fill it out in the office.

Sports/Preparticipation Physicals.

MSHSL physicals (Minnesota) can be performed at the clinic. 
WIAA physicals (Wisconsin) can be performed with approval from the corresponding school nurse.
NCAA physicals (Nation wide) cannot be performed at this time.

Please print off the corresponding form found at the bottom of this page and fill out the personal information and questionnaire prior to coming for the exam. You can also show up 10 minutes before the scheduled exam to fill out the section in person.
